Vision Screening Letters

Here are prewritten letters for schools participating in a vision screening program.  The Vision First preschool or student report form should be sent with the screening letter.

For Preschool

All vision screening programs need a letter that is given to parents as a disclaimer before a vision screening is done. 

 

Important note

The Kids Eyes Count Campaign needs to be approved by your school or school district before materials are distributed. 

Please read the Proposal for Schools and follow the A+ Action Plan for Schools.  We hope you will find these letters helpful, informative, and easy to use. 

 

Here are simple directions

  • Choose Preschool or Grades K-12.
  • Choose the "universal" or "Illinois only" screening letter depending on where you live.
  • Insert the date, your name, your school, the school’s phone number, and approval information in the blue/gray sections.
  • Print your copies. 
  • Distribute the letters and attachments as shown in the A+ Action Plan for Schools.  We made it simple for you.  Each letter has its name in the lower left corner with the name of the attachments included.

Parents need to be informed that a vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor.
